Employers can review current activity as well as prior months’ reports via their secure portal on our website at any time.

COBRA Notices

Notices can be requested by the employer via the website, or utilizing electronic data feeds.

NOTICES INCLUDED

GENERAL NOTICE

to newly enrolled, active employees and if applicable, spouse, with USPS proof of mailing.

ELECTION NOTICE

Initial Qualifying Event Notice and Election Form to qualified beneficiaries with USPS proof of mailing.

ADDITIONAL NOTICES

COBRA Extension Notices due to second qualifying events and Social Security disability.  And, Insufficient Premium Letter sent to COBRA participant whose underpayment is less than or equal to 10% of the total premium due, but no more than $50.00

EARLY TERMINATION

Early Termination Letter if COBRA coverage is terminated early for any reason.
